Job Overview:
We are seeking a compassionate and dedicated Social Care Worker to join our team and support individuals in need of care and assistance.

The ideal candidate will be committed to making a positive difference in the lives of those who require support in areas such as personal care, daily activities, and overall wellbeing.
As a Social Care Worker, you will be a crucial part of our team, providing person-centered care to individuals.

Key Responsibilities:
Provide direct care and support to individuals, ensuring their physical, emotional, and social needs are met.
Support individuals in achieving their goals and improving their quality of life.
Encourage independence and promote choice and dignity for all service users.
Develop and implement care plans in collaboration with the care team and individuals.
Monitor and report on the wellbeing and progress of individuals, documenting all care provided.
Work closely with other healthcare professionals, families, and support teams to ensure integrated care.
Participate in training and development activities to stay current with best practices in social care.
Ensure safeguarding and health and safety policies are followed at all times.
Requirements:

Previous experience in a social care role is essential.
A genuine passion for working with vulnerable individuals and making a positive impact on their lives.
Strong communication and interpersonal skills.
Ability to work both independently and as part of a team.
A commitment to upholding the dignity, rights, and privacy of individuals.
Knowledge of safeguarding procedures and a commitment to promoting a safe and supportive environment.
Flexibility and adaptability to meet the varying needs of service users.
At least 2 years working in a social care background, emphasis working within the homeless sector
Computer literacy essential
Knowledge of the PASS system.
• Knowledge of the PASS system is an advantage.
• Flexibility to work across shifts as required.
