Summary The Health & Social Care Assistant is responsible for delivering designated aspects of residents' care and support under the direction of the Clinical Nurse Manager (CNM) and in collaboration with multidisciplinary professionals, based on individual resident needs and care plans. The role involves working as part of a healthcare team committed to person-centred care, promoting independence, and delivering high-quality services to residents. Key Responsibilities To Provide assistance and support with Activities of Daily Living (ADLs), ensuring residents' comfort, dignity, and wellbeing with a specific emphasis on social, emotional and physical wellbeing. · Assist residents with rehabilitation programmes as directed by healthcare professionals and accurately report progress to CNM. · Work collaboratively with Clinical Nurse Managers and multidisciplinary teams to ensure residents receive coordinated and effective care. · Monitor and report any changes in residents' condition or wellbeing to the CNM/Nurses. · Maintain accurate records and documentation in accordance with organisational policies and procedures. · Promote a safe, respectful, and supportive environment for all residents. Requirements · Minimum of 2 years' experience working in a similar role or within a residential care setting. · QQI Level 5 qualification in Healthcare Support or a related discipline is desirable. · Excellent communication and interpersonal skills. · Ability to work effectively as part of a multidisciplinary team. · A compassionate, patient-centred approach to care. · Good understanding of resident confidentiality, safeguarding, and professional boundaries. Skills and Competencies · Strong teamwork and collaboration skills. · Ability to follow care plans and instructions from CNM’s · Empathy, compassion, and respect for residents' dignity and independence. · Good organisational and time-management skills. · Flexibility and adaptability in a dynamic healthcare environment.
